Friends and family are part of the healing process. However, due to the outbreak of the novel coronavirus COVID-19, we must take extra steps to protect our patients, staff, visitors and the community.

Due to the COVID-19 Pandemic and the State Public Health Officer Order of August 5th, 2021:

Effective August 11th, 2021 

Visitation will be limited to the following:

  1. Visitors that are fully vaccinated*
    1. COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided, and date last dose administered); OR
    2. a photo of a Vaccination Record Card as a separate document; OR
    3. a photo of the client's Vaccination Record Card stored on a phone or electronic device, OR
    4. documentation of COVID-19 vaccination from a health care provider; OR
    5. digital record that includes a QR code that when scanned by a SMART Health Card reader displays to the reader client name, date of birth, vaccine dates and vaccine type.
  2. Visitors that can provide documentation of a negative COVID test taken within the previous 72 hours 

  3. All visitors, regardless of vaccination status or test result, must:
    1. wear a well-fitting face mask upon entry and at all times within the facility
    2. wear any other personal protective equipment (PPE) while in the patient's room that facility personnel deem appropriate to the situation; and
    3. physically distance from health care personnel and other patients/residents/visitors
    4. must stay in the patient’s room for the duration of the visit

*"Fully Vaccinated" means individuals who are considered fully vaccinated for COVID-19: two weeks or more after they have received the second dose in a 2- dose series (Pfizer-BioNTech or Moderna or vaccine authorized by the World Health Organization), or two weeks or more after they have received a single-dose vaccine (Johnson and Johnson [J&J]/Janssen). COVID-19 vaccines that are currently authorized for emergency use.

Debido a la pandemia COVID-19 y la orden del funcionario de salud pública estatal del 5 de agosto de 2021:

Vigente a partir del 11 de agosto de 2021

Las visitas se limitarán a lo siguiente:

  1. Visitantes que han sido completamente vacunados*
    1. tarjeta de registro de vacunación COVID-19 (emitida por los Centros para el Control y Prevención de Enfermedades del Departamento de Salud y Servicios Humanos o Tarjeta Amarilla WHO) que incluye el nombre de la persona vacunada, el tipo de vacuna proporcionada y la fecha de la última dosis administrada); O
    2. una foto de una tarjeta de registro de vacunación como documento separado; O
    3. una foto de la tarjeta de registro de vacunación del cliente almacenada en un teléfono o dispositivo electrónico, O
    4. documentación de la vacuna COVID-19 de un proveedor de atención médica; O
    5. registro digital que incluye un código QR que cuando es escaneado por un lector de tarjetas de salud muestra al lector el nombre del cliente, la fecha de nacimiento, las fechas de la vacuna y el tipo de vacuna.
  2. Visitantes que pueden proporcionar documentación de una prueba COVID negativa realizada dentro de las 72 horas anteriores 

  3. Todos los visitantes, independientemente del estado de vacunación o el resultado de la prueba, deben:
    1. usar una mascarilla facial bien colocada al entrar y en todo momento dentro de la instalación.
    2. usar cualquier otro equipo de protección personal (PPE) que el personal de la instalación considere apropiada para la situación mientras esté en la habitación del paciente; y
    3. mantener distancia física del personal de la salud y de otros pacientes / residentes / visitantes
    4. debe permanecer en la habitación del paciente durante la duración de la visita

*"Totalmente vacunada" hace referencia a las personas que se consideran completamente vacunadas contra COVID-19: dos semanas o más después de haber recibido la segunda dosis de una serie de 2 dosis (PfizerBioNTech o Moderna o vacuna autorizada por la Organización Mundial de la Salud), o dos semanas o más después de haber recibido una vacuna de dosis única (Johnson y Johnson [J&J] / Janssen). Vacunas COVID-19 que actualmente están autorizadas para uso de emergencia.
